The duelists' duel disks relay the duel information to the D-Gazers, which then show the duel pretty much the same way the holograms did, but with slightly more "interaction" with the real world.
The Arclight family doesn't use regular D-Gazers. Instead, in the case of III, IV, and V, they have a tattoo which forms around the left eye and turns that iris a different color, as if it was covered by colored glass. The tattoos are able to connect with the AR network and other D-Gazers just like any other D-Gazer."
Obviously, as is, IV's D-Gazer is going to do diddly-squat in the City other than look pretty cool. So Lachesis modified it a bit so it doesn't only show AR, but how the world around him really is. This means he can see anything invisible to the average human, through illusions, and the true physical nature of people brought in by the Porter (meaning, if he looked at a Transformer, he'd see their humanized form but also a multi-story alien robot on top of it). This applies to shape-shifters too, not just beings changed by the Porter.
This only works in person and it only works while his tattoo is active, which won't be all the time. And it could probably be blocked by psychic powers or if you just really don't want it to work on your character, well, sometimes Lachesis' powers don't work perfectly. OH YEAH and I should mention this won't allow him to see secret identities and whatnot so you costumed types are safe.

Are they a shapeshifter? If being able to extend his limbs and turn into a puddle counts, then yes.
Is it okay for IV to see through those powers? Certainly.
Was this character's physical being changed by the Porter some way, and if yes, how? No
Is it okay for IV to see what they should really look like? If he's human again? Yes.
